At Rhema Home Health Care, our goal is to help you or your loved one get back to your previous state of health and achieve independence in the home after an illness. We are your partner in the recovery journey and transition from the hospital back to your home.
Our visiting nurses will work with you to develop a plan of care customized to your own needs. They will come up with a schedule of visits that will ensure that you get adequate support in services such as medication management and teaching, teaching on how to recognize and report abnormal symptoms, wound care, vital signs, weight checks, etc.
By fulfilling the above, we will help our clients to avoid unnecessary Emergency Room visits, prevent re-hospitalization, prevent complications, and enable our clients to have an overall faster recovery.
